DESCRIPTION
OF MEDIATION FORMS
The
General Information From should be completed by
all potential clients. This form assists the
mediator in determining whether mediation is
appropriate for resolving the dispute.
The
Client Information From is for use by the
mediator during divorce and family mediations.
Information provided on this form is strictly
confidential and privileged.
The
Mediation Survey Form is designed to obtain
feedback from clients about the mediator and the
process of mediation. All clients are encouraged
to complete this form after the conclusion of their
mediation and return it to GMRS by mail, fax or email.
